Description

To report accurate rates, many HEDIS measures, and other clinical performance measures, require the collection of data from medical records as the primary or supplemental source of provider diagnostic and procedural information. This often requires provider office site visits from registered nurses or skilled medical record abstractors to obtain the critical information to combine with claims and other encounter data, to accurately report final HEDIS rates.

HYBRIDHelp is Q Mark’s laptop and workstation software tool used for medical record review of HEDIS hybrid measure abstraction and for customized quality improvement studies. HYBRIDHelp fully integrates with HEDIS for administrative reporting software for aggregate reporting of HEDIS results.

Extensive and Complete Data Edits Ensure High Quality Data Collection

Because HYBRIDHelp incorporates all data edits within the software application, the user is informed when data are invalid and outside of the measurement parameters, or that it validates compliancy for the measure or numerator for a measure while still in the medical record. Numerous interactive messages guide the abstractor through each measurement’s data collection process, ensuring that the required data are collected before the abstractor leaves the clinical location.

Complete Remote Data Synchronization

HYBRIDHelp data collection often requires abstractors to work remotely and off-line. HYBRIDHelp allows all abstractors to carry an entire client's database (or several databases) with them on a laptop computer. At the conclusion of data collection each day, the abstractors upload the data over a secure VPN connection to the central server located at the Q Mark office. Simultaneously, data is downloaded that contains all other abstracted data. This allows abstractors, project managers, and clients to access real-time data regarding provider site scheduling, project status, and a variety of additional status reports.

The software that Q Mark develops is used by its own staff and is a robust, refined, easy-to-use, Windows-based system with intuitive wizard-type interfaces.
